WAC 110-300-0475(2)(a)(vi) - It was determined that 3 staff persons were aware of an incident of inappropriate touching between two children that occurred on 10/18/2024. The 3 staff persons were center director Rachel Cobb and teachers Rene Acton and Alfari Martinez Quiroga. None of the staff persons reported this incident to law enforcement or the department at the first opportunity or within 48 hours as required. Complaint is 'valid' for failure to report. WAC 110-300-0475(3)(b) - It was determined that the center director, Rachel Cobb, did not submit a written incident report to the department within 24 hours following a situation that may put children at risk.
